Understand every section of the PhilHealth Member Registration Form.
Write your full name exactly as it appears on your PSA Birth Certificate. Use BLACK ink. Do not abbreviate names. For contact information, provide an active mobile number and email address.

- Misspelling the middle name or omitting it entirely
- Writing an incorrect birth date — double-check against your Birth Certificate

Complete both residential and permanent address fields. Include house number, street, barangay, city/municipality, province, and ZIP code. If residential and permanent addresses are the same, write "SAME AS ABOVE" in the permanent address section.

List each dependent's full name, date of birth, and relationship to you. Qualified dependents include your legal spouse, children under 21, children over 21 with disabilities, and parents aged 60 and above who are not PhilHealth members.

Tick ONE box that matches your current status. "Employed" includes government and private employees. "Self-employed" includes professionals and business owners. "Voluntary" is for those not covered by other categories. "OFW" is for overseas Filipino workers. "Sponsored" is for those covered by LGU or NGO sponsorship. "Indigent" requires DSWD certification. "Lifetime" applies to retirees who have paid at least 120 monthly contributions.

Read the declaration carefully. Sign your full signature — the same signature that appears on your valid ID. Write the date of signing in DD/MM/YYYY format. If you are representing someone else, indicate your relationship to the member.

⚠️ Common PMRF Mistakes to Avoid
These are the most frequent errors that cause PMRF rejection or processing delays.
Many applicants forget to sign the Declaration section. Without your signature, the PMRF is invalid and will be rejected.
Ticking "Voluntary" when you are employed (or vice versa) leads to incorrect contribution billing and may delay your membership activation.
Your birth date must match your PSA Birth Certificate exactly. Even a one-digit error can cause your record to be flagged.
If you add dependents in Section III, you must attach supporting documents for each dependent (Birth Certificate, Marriage Certificate, etc.).
Leaving the permanent address blank or writing an incomplete address (no barangay, no ZIP code) can delay processing.
PhilHealth updates the PMRF periodically. Always download the latest version from philhealth.gov.ph. Using an old form may result in rejection.
