What is the PhilHealth MDR and why do you need it?

The Member Data Record (MDR) is the official PhilHealth document that proves your membership status, lists your qualified dependents and shows your PhilHealth Identification Number (PIN). Hospitals require a current MDR before they can apply PhilHealth benefits to your bill, and employers often ask for one during onboarding or contribution reconciliation.

Since PhilHealth digitised its services, the fastest way to get your MDR is through the Member Portal at philhealth.gov.ph. You log in, click the MDR section and download a PDF in about two minutes.

        What is a PhilHealth MDR?
        The Member Data Record (MDR) is an official PhilHealth document that summarises your membership information. It shows your PhilHealth Identification Number (PIN), full name, date of birth, membership type, coverage status, qualified dependents and last posted contribution period.
        How do I download my MDR online?
        Log in to the PhilHealth Member Portal at philhealth.gov.ph using your registered email and password. Once logged in, navigate to the MDR section on your dashboard and click Print or Download.
        Can I get my MDR without a portal account?
        Yes. You can request a printed MDR at any PhilHealth Local Health Insurance Office (LHIO) or branch by presenting a valid government-issued ID.
        Is the MDR the same as a PhilHealth ID?
        No. The MDR is a data summary document, not an identification card.
        What documents do I need to get my MDR at a branch?
        Bring at least one valid government-issued ID such as a UMID, passport, driver's licence, PhilID, PRC card or voter's ID.
